The CMO Process
Children and families in the CMO are referred by a state contracted administrative management company (CSA) that assesses their needs. The CSA can be reached at 1-877-652-7624.
Once referred, a family is assigned a Care Manager who develops an Individual Service Plan (ISP) based on the family’s goals, strength, and needs. A Child and Family Team is formed and includes anyone who plays a significant role in the child or family’s life. Such a team might include friends, relatives, pastors, teachers, or community service providers. The team’s purpose is to support and assist the family in attaining the ISP goals.
