CHILDREN'S SYSTEM OF CARE & WRAPAROUND

When you call Performcare they will ask you a variety of questions to determine what types of services are appropriate for your child and family. If your child is experiencing issues that require a more in-depth, immediate assessment, they will send a Clinician from Mobile Response to your home to meet with you and your child.

WHAT IS THE WRAPAROUND MODEL?

PerformCare is the single point entry for all children, youth, and young adults entering the New Jersey System of Care. We follow a service delivery model called Wraparound that is built upon a strong foundation of core values and guiding principles.

PRINCIPLES

  • All children who need services should receive the same accessibility to services
  • Availability and access to a broad, flexible array of community-based services and support for children, and their families and caregivers, to address their emotional, social, educational, and physical needs, should be ensured.
  • Services should be individualized in accordance with the unique needs of each child and family
  • Services should be guided by strenth-based, wraparound service planning process and s service plan that is developed in true partnership with the child and family.
  • Services should be delivered in the least restrictive settings that are clinically appropriate.
  • Treatment outcomes for children and families should be quantified.


LEVELS OF CARE

When a parent or guardian refers a child to the System of Care, a clinical professional goes to the child’s home and meets with the child and the family to complete an assessment. The assessment determines which of the three levels of care the child and family should receive.

The least intensive level of care provides access to specialized out-patient services that are managed by the Contracted System Administrator in consultation with the parent/guardian and the out-patient therapist. Services are intended to be short-term and help families move forward through a difficult period. This level is appropriate for youth who are struggling in one area (such as communication, school performance, socialization skills, age-appropriate behavior challenges, etc).

Youth with  moderately intense and high intensity needs, who display concerns in multiple life dimensions (school, home, community) may receive linkage and monitoring of services and supports through the Care Management Organization (CMO). Youth who are assessed to have high intensity needs may be involved in multiple systems (school, court, probation, DCP&P, the Family Support Organization, etc.) and the Care Manager acts as the point person for all system partners to ensure that everyone is on the same page and working towards the goals identified by the youth and family. Care Managers help families create Child and Family Teams (CFTs) that are led by the parent/guardian with the help and guidance of the Care Manager. The Child and Family Team assists the child and family in creating strategies to resolve the challenges they face. Specialized services and support to be used are determined in the Child and Family Team meetings and may include therapy, community-based mentors, engagement in social and recreational activities, re-engagement with school, employment, etc. The Child and Family Team helps the family identify and develop a support system that will be sustained long after CMO services end.

WRAPAROUND-WHAT IS IT? Detailed Explanation


FAMILY VOICE AND CHOICE: 

Family and youth/child perspectives are intentionally elicited and prioritized during all phases of the wraparound process. Planning is grounded in family members’ perspectives, and the team strives to provide options and choices such that the plan reflects family values and preferences.


TEAM BASED: 

The wraparound team consists of individuals agreed upon by the family and committed to the family through informal, formal, and community support and service relationships.


NATURAL SUPPORT: 

The team actively seeks out and encourages the full participation of team members drawn from family members’ networks of interpersonal and community relationships. The wraparound plan reflects activities and interventions that draw on sources of natural support.


COLLABORATION: 

Team members work cooperatively and share responsibility for developing, implementing, monitoring, and evaluating a single wraparound plan.The plan reflects a blending of team members’ perspectives, mandates, and resources.The plan guides and coordinates each team member’s work towards meeting the team’s goals.


COMMUNITY BASED: 

The wraparound team implements service and support strategies that take place in the most inclusive, most responsive, most accessible, and least restrictive settings possible; and that safely promote child and family integration into home and community life.


CULTURALLY COMPETENT: 

The wraparound process demonstrates respect for and builds on the values, preferences, beliefs, culture, and identity of the child/youth and family, and their community.


INDIVIDUALIZED: 

To achieve the goals laid out in the wraparound plan, the team develops and implements a customized set of strategies, supports, and services


STRENGTHS BASED: 

The wraparound process and the wraparound plan identify, build on, and enhance the capabilities, knowledge, skills, and assets of the child and family, their community, and other team members.


PERSISTENCE: 

A wraparound team does not give up on, blame, or reject children, youth, and their families. When faced with challenges or setbacks, the team continues working towards meeting the needs of the youth and family and towards achieving the goals in the wraparound plan until the team reaches agreement that a formal wraparound process is no longer necessary.


OUTCOME BASED: 

The team ties the goals and strategies of the wraparound plan to observable or measurable indicators of success, monitors progress in terms of these indicators, and revises the plan accordingly
